(a) is for the characterisation of the following:

(i) chromosome translocations t(4;14), t(14;16), t(14;20);

(ii) 1q gain;

(iii) 17p deletion; and

(b) is for a patient with clinical or laboratory evidence, including morphological features, of plasma cell myeloma

Applicable only once per lifetime
